This thing is an extension for the fully printed version, where the metal M6 screw and nut is replaced by a printed spindle and printed insert with female thread. It extends the length of the spindle by 5mm, which can be crucial with you need the utmost opening of the vise for your application. It was explicitly requested by other users on printables.com.
