Summary
Given the games rarity and age I would be surprised if anyone ever ends up using this, but as my first attempt at designing an insert I'm happy to publish it.
Print one of each of the files excepting the player trays, of which 5 should be printed.
This was not designed to support sleeved cards and I suspect they would not fit properly.
